Before Orientation

All first-year students will have a number of forms to complete and submit to the MEDC-111 Canvas course by Wednesday, July 30, 2025. Copies of the forms, submission pages, and detailed instructions will be made available to you in Canvas on July 21, 2025.  

A list of forms can be found below. Please note that this list is subject to change, and some forms are specific to site. Canvas should be considered the primary point of reference for accessing these forms.

CPSS License Application

The following forms must be filled out and returned to the College of Physicians and Surgeons of Saskatchewan (CPSS) in order to receive your educational license. If you have any questions or concerns, contact the CPSS office at cpssreg@cps.sk.ca or call 306-244-7355.
