Welcome to first-year students to the University of Saskatchewan College of Medicine! Please familiarize yourself with the materials below before orientation.

All first-year students must complete the following forms and submit to the MEDC 111 course on Canvas by Wednesday July 31st, 2024. You will have access to the forms on Canvas on Friday, July 26th, 2024.

2. Audiovisual Recording Consent – CLRC/RLC/DSC
3. Consent to Record – UGME
4. Release of Information Consent Form
5. SHA Confidentiality Agreement
6. Student Photo Consent Form
7. Student Photo Upload
8. Work-Based Learning Consent and Agreement (WCB)
9. Consent to use Image Recording (Regina Students, due July 30) 
10. Student ID Hospital Badges (Saskatoon Students only, Regina will share process in person)

 

Other Forms/Training

1. Personal Data Sheet (Complete on One45 and upload PDF on Canvas) – due August 5
2. CPR/BLS/AED Training Certificate – due September 3 

The following forms must be filled out and returned to the College of Physicians and Surgeons of Saskatchewan (CPSS) in order to receive your educational license. If you have any questions or concerns, contact the CPSS office at cpssreg@cps.sk.ca or call 306-244-7355.
