Access to a form for release of student information.
A form entitled "Release of Information Authorization" is available online for students' use. This form is used if the student requires authorization for release of information.
These forms will be kept on file and in a database at Student Central.
If a staff or faculty member encounter someone stating they have authorization for a release of student information, please call Student Central at (306) 966-1212 or email at askus@usask.ca for verification of authorization.
